The patient is a known case of hypertension for more than 10 years, fairly compliant to amlodipine 5mg tab OD, s/p CVD Infarct (2017,2021); came in with 3 months history of dizziness and body weakness; work up was done which revealed premature ventricular contractions (PVCs) on 12LECG; coronary angiogram was done which revealed 3-vessel disease hence patient was advised coronary artery bypass grafting (CABG).
PMH: hypertensive; non- diabetic
PSHx: non-smoker, non-alcoholic beverage drinker
PHYSICAL EXAMINATION:
VS 120/80, 105, 24, 36.6, 99% at room air
GCS 15
Not in cardiorespiratory distress, awake and conscious
SCE NR; CBS
AP, normal rate, irregular rhythm, no murmur
Flabby abdomen soft, NABS NM
Full and equal pulses (-) edema
Medications:
Valsartan 160mg tab OD
Trimetazidine 35mg tab BID
Clopidogrel 75mg tab OD
Apixaban 5mg ½ tab BID
Bisoprolol+Amlodipine 5mg/5mg tab OD
Atorvastatin 80mg tab ODHS
