This abstract synthesizes insights from Assessments 1, 2, and 3, emphasizing the importance of interprofessional team partnerships in strengthening hypertension management. Systems Theory provides the foundation for understanding how interconnected provider roles contribute to quality, efficiency, and patient-centered care. Literature supports the value of coordinated interventions, including shared medical appointments, nutrition counseling, and remote monitoring, all of which enhance patient outcomes and reduce fragmentation (Ganetsky et al., 2020; Soltani et al., 2020). Interprofessional collaboration fosters innovation through structured communication, case reviews, and shared care pathways (Irazola et al., 2025). Furthermore, integrating patient preferences into decision-making promotes ethical practice and strengthens adherence (Gurmu, 2022). Overall, applying scholarship and evidence-based principles through interprofessional teamwork creates sustainable improvements across the hypertension care continuum (Jason & Bobak, 2022).
